TVT represents nowadays the gold standard for surgical treatment of female stress incontinence with excellent cure rates. Some problems regarding retropubic hematomas, bladder perforation, vessel or nerve injuries or tape erosion are reported. TOT is a new interesting approach that seems to warrant similar cure rates, without the necessity of cistoscopy and with no risk of bladder perforation, bowel, nerves or large vessel injuries. Results of 146 patients with follow up superior to 6 months are presented:TVT (72 cases) and TOT (74 cases).  This randomized study confirms that cure rate of the 2 procedures are similar even if the follow up is short. The time necessary to complete the operation is shorter with TOT than with TVT.
