Summary: To assess the reliability of a presurgical evaluation of endometrial cancer in selecting low and high risk population in which different surgical approach can be proposed. Seventy-four patients with endometrial carcinoma underwent a presurgical evaluation of grading ( endometrial biopsy ) and myometrial invasion ( transvaginal sonography ) in 38 ploidy was assassed ( cytofluorometry ). We were able to identify 19/24 (79%) low risk cases ( G1/M0-M1) and 47/50 (90%) high risk cases ( G2-G3/M2 ). The possibility to identify preoperatively the risk of node metastasis justifies the choice of different surgical approaches , i.e. vaginal surgery plus operative laparoscopy in low risk cases.
